Tips of Penoscrotal IPP

Steven K Wilson, MD, FACS, FRCS, Institute for Urologic Excellence, La Quinta CA, USA

Introduction: A transverse high scrotal incision is utilized for maximum corporal exposure. Tips of the various steps to implant a 3 piece inflatable penile prosthesis are outlined by a physician with 40 years of experience
Case Presentation: not applicable
Protocol: Proper deployment of the “Scott “ retractor is an aid to maximizing exposure. Sharp dissection of the Dartos muscle and lateral dilatation of the Corpus Cavernosa are recommended tips for efficient implantation of the cylinders. It is important to insure dilatation proximally to the ischial tuberosity to prevent improper cylinder sizing. The retroperitoneal reservoir insertion is illustrated with the aid of a laprascopic camera. Partial inflation, a closed suction drain and a pressure dressing of the scrotum are employed to prevent scrotal hematoma.
Outcome: Not applicable
Discussion: Following this step by step instruction guide should allow physicians to efficiently place an inflatable penile prosthesis. The laparoscopic visualization of the transversals fascia piercing should remove the mystery surrounding “blind” reservoir placement.
