This video illustrates a new technique for achieving additional straightening during implantation of an inflatable penile prosthesis for Peyronie’s Disease. After making the corporotomy, dilating the corpora and before the implantation of the cylinders a distinct surgical maneuver is performed to weaken the Peyronie’s plaque. A sharp instrument such as a hook bladed knife, a scissors or a cavernotome is used to vigorously scratch the plaque. It is thought by the authors that this incision of the fibrotic plaque weakens the plaque making it more amenable to the straightening accomplished by implantation of the cylinders. The authors contend that if more straightening is needed the scratching of the plaque makes modeling easier to accomplish. The scratch technique is illustrated by surgical video, medical illustration and a unique intra corporal camera.
