Traditional placement of the inflatable penile prosthesis reservoir into the space of Retzius can be associated with bladder, bowel and vascular injuries. Men with a history of pelvic surgery can have the reservoir placed between the rectus abdominis muscle and the transversalis fascia, while men with virgin pelves can have the reservoir placed between the transversalis fascia and the peritoneum, which avoids a palpable reservoir. In this video, we present Dr. Perito’s technique of inserting the reservoir posterior to the abdominal wall musculature, which can be performed via an infrapubic or penoscrotal incision.
